Truemed & HSA/FSA Payments

Truemed: Use Your HSA/FSA for Fitness

Truemed connects you with a licensed provider to get a Letter of Medical Necessity - unlocking HSA/FSA payments at 30+ gyms and fitness brands for a one-time $30 fee.

What Is Truemed?

Truemed is a healthcare platform backed by $34M in Series A funding from Andreessen Horowitz (a16z). It solves a specific problem: the IRS requires a Letter of Medical Necessity (LMN) before you can use HSA or FSA funds for fitness expenses. Getting an LMN traditionally means a doctor visit, a copay, and waiting for paperwork.

Truemed streamlines this into an online health survey reviewed by a licensed provider. If you qualify, you get your LMN in minutes to 24 hours - and can immediately use your HSA/FSA card at partner merchants. If you do not qualify, you are not charged.

How Truemed Works (Step by Step)

  1. 1

    Start at a partner merchant

Visit a Truemed partner website (gym, equipment brand, or supplement company). Look for "Pay with HSA/FSA" at checkout.

  2. 2

    Complete the health survey

Answer questions about your medical history, current conditions, medications, and health goals. Takes 5-10 minutes.

  3. 3

    Provider reviews your profile

A licensed provider (MD, DO, NP, or PA) evaluates your health history and determines if the expense is medically necessary for your specific condition.

  4. 4

    Get your LMN

If approved, you receive a Letter of Medical Necessity with your diagnosis, ICD-10 codes, and the provider's determination. Valid for 12 months. You are charged $30.

  5. 5

    Pay with HSA/FSA

Enter your HSA or FSA card at checkout. The purchase is processed as a qualified medical expense. Keep your LMN on file for your HSA administrator.

The $30 fee itself is not HSA-eligible (it is a service fee, not a medical expense). But the savings from using HSA funds for the underlying purchases far exceed the $30 cost.

Truemed vs Flex

Flex is the other major HSA/FSA fitness payment platform. The biggest difference: gym network. Here is a side-by-side comparison.

Bottom line: If you are an Equinox member, Flex is your only option. For everyone else, Truemed offers broader coverage at a lower cost. See our gym membership eligibility page for more on qualifying conditions.

Who Qualifies?

Truemed's licensed providers evaluate your health profile against IRS requirements. Common qualifying conditions include:

Obesity (BMI 30+)
Type 2 Diabetes
Cardiovascular Disease
Hypertension
Depression & Anxiety
Chronic Pain
Metabolic Syndrome
Sleep Disorders
Musculoskeletal Disorders
PCOS

This is not exhaustive. The provider evaluates your complete health history. If they determine exercise-related expenses are medically necessary for your condition, you qualify.

Tips for Getting Approved

  • Be thorough on the health survey. Include all diagnosed conditions, medications, and relevant medical history.
  • Mention specific conditions rather than general statements. "Diagnosed with obesity E66.01" is stronger than "trying to lose weight."
  • Include your doctor's recommendations if your physician has recommended exercise for your condition.
  • Have recent lab results available - blood work, A1C levels, or other diagnostics that support your condition.

Frequently Asked Questions

What is Truemed?
Truemed is a healthcare platform that connects consumers with licensed providers to obtain Letters of Medical Necessity (LMNs) for health and fitness expenses. With a valid LMN, you can pay for qualifying products and services with HSA or FSA funds. Truemed raised a $34M Series A led by a16z in 2025.
How much does Truemed cost?
Truemed charges a one-time $30 fee for the LMN evaluation. If the provider determines you do not qualify, you are not charged. There are no monthly fees, no percentage-based charges, and no hidden costs. The $30 covers 12 months of eligibility.
How long does the Truemed process take?
Most people complete the health survey in 5-10 minutes. Provider review typically takes minutes to 24 hours. Once approved, you can immediately use your HSA/FSA card at partner merchants.
What if I do not qualify through Truemed?
If the licensed provider determines your health profile does not meet the criteria for a Letter of Medical Necessity, you are not charged the $30 fee. You can also try getting an LMN directly from your primary care physician.
Is Truemed legitimate?
Yes. Truemed works with licensed healthcare providers (MDs, DOs, NPs, PAs) who evaluate your health history and issue clinically appropriate LMNs. The company raised $34M from Andreessen Horowitz (a16z) and partners with major fitness brands. LMNs issued through Truemed are valid for HSA/FSA reimbursement.
Can I use Truemed for Equinox?
No. As of February 2026, Equinox partners exclusively with Flex for HSA/FSA payments. Truemed covers 30+ other gym brands including Barry's, CorePower Yoga, Life Time, and 24 Hour Fitness.
Does Truemed work with FSAs too?
Yes. Truemed supports both HSA and FSA payments. The same Letter of Medical Necessity applies to either account type. The LMN process is identical regardless of which tax-advantaged account you use.
What happens after 12 months?
Your Letter of Medical Necessity expires after 12 months. To continue using HSA/FSA funds, you will need a new evaluation and LMN. Some partner merchants handle renewals automatically through Truemed.
